Student Immunization Requirements

Every year schools are required to assess all students’ immunization records to determine compliance with the Wisconsin Student Immunization Law.  Students have until the 30th school day to provide the appropriate documentation of immunization or a signed waiver.  Starting on the 31st school day, non-compliant students may be excluded from school, in order to remain compliant with the law.

Student Immunization Record Form - Required form to be completed at the beginning of the school year with student's immunization history.  This is due by the 30th school day.   Please complete and print form, then drop it off at your child's school office.

Student Flu Vaccine Clinics

Student flu vaccine clinics are scheduled each fall. This year Coloma Elementary's clinic is on October 9th and Oxford Elementary, Westfield Elementary, and the Middle and High School's clinic is on October 22nd.

If you wish for your child to receive a flu vaccine, please fill out the corresponding form found below, print it, and return it to your child's school office.

If you do not wish for your child to receive the flu vaccine at the school vaccine clinic, you do not need to return any forms.  Only children who have completed consent forms will receive any vaccines.
